The problem and who it affects, for example screen reader or keyboard only users. 2. The guideline it maps to, such as 1.4.3 Contrast or 2.1.1 Keyboard. 3. Severity: blocker, major, or minor. 4. A concrete fix, with the exact value where relevant, such as a contrast ratio or target size. ## Quality bar - Cover contrast, keyboard access, focus order and visibility, target size, labels and names, and status messages. - Reference specific elements, not vague areas. - Do not invent issues; if something passes, note it. ## Output A punch list sorted by severity, plus the top three fixes to ship first.
